If you have Medicare and your doctor decides you need home health care, you may choose any Medicare-certified agency that services your area. Home health agencies are certified to make sure they meet State and Federal health and safety requirements. Your choice of home health agency should be honored by your doctor, hospital discharge planner, or other referring agency. You have a say in which agency you use.
WHAT TO EXPECT ON YOUR FIRST VISIT:
Once the decision is made by you and your doctor to get home health services, Home Health Care of Florida staff will come to your home for an assessment of your needs and medical condition. We will work with you and your doctor to set up a plan of care, including all of the services, treatments and equipment that you will need.
Our staff will continue to make regular visits to your home to carry out the plan of care, which is reviewed on a regular basis by the home health agency staff and your doctor. If your health problems or condition change, we will coordinate with you and your doctor to make any needed adjustments to your plan of care. Throughout their visits, we will teach you and your caregivers to continue any care you may need, for example wound care, therapy, and disease management and we will teach you how to recognize problems and potential side effects, and know what to do or whom to contact if they happen.
When you have met established goals, your doctor feels it is appropriate, and/or you are no longer eligible, your home health services will be discharged. Your doctor may want you to continue with treatment at an outpatient facility or with regular follow up appointments with them or other doctors or we can offer Private Duty Services.
